Scope of Work and Materials Description:

1. Dismount all van rear passenger seat assembly and re-install one collapsible original  (jump seat) at the rear portion of the driver and passenger side to serve as a doctors seat.
2. Fabricated nurse and aide seat at the right side of the van (made of plywood with carpet,with cushion and leatherrette seats)
3. Install 1/4 thick marine plywood and cover floor with aluminum checkered plate.
4. Ambulance stretcher, fully collapsible (Imported) 8 wheels w/ foam & safety belt
5. Install Ambulance stretcher assembly, Oxygen tank holder stainless steel locally assembled.
6. BEACON type warning light, Red/Blue lens combination, strobe type with holder bracket.(option to all red/all green)
7. Siren and Speaker with public address (PA) system including handheld microphone.
8. Fire Extinguisher, protable 5 lbs. Capacity with holder, wall mounted.
9. Portable Flashlight, 2 battery, Dry cell size D.
10. One portable medical oxygen tank with content and holder 20 lbs. Capacity.
11. Window curtain made in thailand, cotton  fabric, LH/RH side and rear medical green color and TINT rear winshield and side windows door side of van – One way green scratch proof & non-fading
12. (A) Full body lenght cabinet, with flexi glass sliding doors.                        (B) Spine Board.
13. Oxygen tank gauge regulator and de-humidifier, with nasal cannul.
14. Minor first aid kit including bag or box, local
15. BP Apparatus, aneroid type, with standard stethoscope.
16. Blood pressure Gauge, wallmounted.
17. Ambubag with resucitator, Adult and Pedia combination. (Imported)
18. Minor Surgical kit, with bag (imported)
19. IV bottle holder, ceiling mounted. Stainless steel shafting.
20. Lettering of word “AMBULANCE”, front and rear portion of van.
21. Stickers includes company logo as specified by the client (maximum of 2 sets logo)
22. Flourescent light
23. Return all excess items, like passenger seats and floor carpet back to customer.
24. LED search light with remote.
